Medical Packet Information

All student-athletes at Limestone University are required to have an annual physical examination and updated medical information. In order to be cleared for athletic participation each student-athlete needs to complete the medical packet.

The South Atlantic Conference, Conference Carolinas and member institutions voted and passed a new rule that before any student-athlete is permitted to practice, compete or take part in out-of-season training a sickle cell test must be completed. This rule applies to all initial and transfer student-athletes. If you have previously taken a sickle cell test then all that is needed is the results.

Effective August 1, 2009, the NCAA requires stricter documentation to confirm the diagnosis and treatment plan for Attention Deficit Disorder/ Attention Deficit Hyperactivity Disorder. It is now required that the “institution should keep on file a record of the student-athlete’s evaluation, diagnosis, the history of treatment, and a copy of the most recent prescription.” This information must be updated annually to include the most recent prescription. We appreciate your assistance in providing the necessary documentation.
 
In the past, a simple copy of the prescription and a note stating the diagnosis was sufficient to meet the NCAA compliance recommendations. This is no longer adequate. Failure of the student athlete to comply may lead to loss of up to a year of eligibility. If a student athlete fails a drug test with a stimulant in their system, having this new documentation available will speed up the process allowing their return to sport. Retrospective gathering of the information will not guarantee the student athlete winning their appeal and they will be withheld from sport until this appeal process is completed.
